Management of Long-Term Conditions for Healthcare Professionals

Designed for healthcare professionals new to General Practice, this module covers political issues, NHS strategies and developments in primary care, giving you invaluable knowledge and an understanding of the key issues that impact your role.

Credits: 30 credits at Level 6.

Why study this subject?


This module will help empower healthcare professionals to learn the foundational knowledge and skills required in the Primary Care setting.

What will I study?


Led by our experienced and supportive staff, this module will cover an introduction to key long-term conditions, monitoring and management in areas such as:

  • asthma
  • chronic obstructive pulmonary disease
  • diabetes
  • dementia
  • cardiovascular disease
  • hypertension.

You'll also learn about a wide variety of topics essential to your role, including:

  • consultations
  • health assessments
  • diagnostic tests
  • health screening
  • long-term condition monitoring and management
  • self-care planning
  • medicine management
  • working with individuals who have a variety of health needs.

The module will also cover:

  • patient as a partner
  • team working and leadership
  • record keeping and IT in relation to clinical systems
  • quality monitoring and assurance
  • ethical and legal issues, negligence and employment law.

How will I be taught and assessed?

You'll benefit from a combination of University teaching and learning in practice with mentor guidance. There are five face-to-face/virtual teaching days that are supported by e-learning material at our Uxbridge and High Wycombe campuses

A variety of teaching methods are used within the programme including group work, self-directed learning, demonstrations and tutorials. Online learning will be used to complement traditional methods of teaching. 

You'll be expected to produce a summative report as part of your assessment which should be based on a patient from general practice where you've been involved in the management of their long-term condition.

The credits from this module can be used towards an honours degree qualification in BSc (Hons) Professional Practice (Healthcare).

What are the course entry requirements?

You must be a qualified, registered Nurse Associate or Health care professional and hold current professional registration on Part 1 of the Professional Register. You will also need to have evidence of having studied at Level 5. You have to be working in a General Practice for a minimum of two days per week and have access to a mentor working in this setting to support you through the programme.
